I accept the DEVICE ON LOAN is from a Teen-Turn partner. I accept full responsibility for all risks arising from possession and / or use of the DEVICE and accept that Teen-Turn and the partner shall not be liable for any direct, indirect, special or consequential damages arising out of or in connection with our possession or use of the DEVICE.

I waive and release any and all claims against Teen-Turn with respect to any and all injury, disability, death, loss or damage to property resulting from possession or use of the DEVICE.

I grant permission to use photographs showcasing my participation and involvement in Teen-Turn for any legal use including but not limited to: online media, broadcast media, print media and social media. Furthermore, I understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.
